Many health plans, accountable care organizations (ACOs), and community-based organizations and partners are working with Medicaid programs to transform health care delivery and transition fee-for-service long-term services and supports (LTSS) to a managed care model known as managed LTSS (MLTSS).
The Goal: To enable individuals with disabilities to live and work in the setting of their choice, maintaining a satisfactory quality of life—while receiving appropriate services to improve health outcomes and reduce costs.
