Integrate Social Determinants into Health Care Delivery and Medicaid ACO Model, 3:30 p.m., case study

Robert W. Seifert, MPA, principal, Center for Health Law and Economics, will discuss the intricacies in identifying social determinants in health care delivery and payment structures, despite their undeniable influence on individual and population health.

Seifert will focus on the social determinants included in the Medicaid MCO models in Massachusetts. His case study format includes insight on how to develop the infrastructure necessary to better collect and integrate information, including how Census and other data sources can aid medical claims data for identification and risk assessment.
